Minors must be accompanied by a parent/guardian, and have a state-issued photo ID that matches their parent/guardian's ID (last name and address) OR a school ID paired with their original birth certificate. 

If the Parent/Guardian's last name is different from the minor's, please bring accompanying documentation to prove guardianship.

For earlobe piercings, if the minor is too young for a school or state-issued ID, please bring their original birth certificate with their accompanying parent/guardian's legitimate ID.
